29 CFR 1926.20(b)(2): The employer did not ensure that frequent and regular inspections of the job sites, materials, and equipment were conducted by a competent person: (a) At the facility employees were exposed to fire hazards, such as but not limited to, exit door was not installed to open in the direction of travel, illuminated exit sign was in wrong location, storage and equipment was stored in the exits. The hazards were created during construction, where the employer failed to ensure that a competent person conducted inspections to identify the hazards existed on or about 6/29/2015.

29 CFR 1910.141(a)(3): The employer did not ensure that all places of employment were kept clean to the extent that the nature of the work allowed. (a)At the facility, on the 2nd and 3rd floors, employees of the Veterans Benefits Administration were exposed to a potential respiratory hazard, in that, the air handler units (AHU), such as but not limited to AHU in mechanical room 224-F had visible, possible fungi growth accumulation on and around the air supply ducts and fans on 7/21/2015.
