29 CFR 1910.22(b)(1): Where mechanical handling equipment is used, sufficient safe clearances shall be allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways shall be kept clear and in good repairs, with no obstruction across or in aisles that could create a hazard.    a.     Warehouse - On or about July 01, 2015, in the warehouse, employees were required to operate powered industrial vehicles. The aisle ways in the southwest corner of the warehouse were not kept clear (blocked by overstock). Employees were thereby exposed to the hazards associated with blocked aisles and passageways.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines, whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.     a.     Production Department - On or about July 01, 2015, Full-time employees and temporary employees were required to operate the Cold Roll Machine / ORT, Model RP90, Serial number 1542 in the production area on a daily basis.  The employer did not have the points of operations completely guarded on Cold Roll Machine / ORT. Employees were thereby exposed to the hazards associated with points of operations.

29 CFR 1910.219(f)(3): Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets.     a.     Production - On or about July 01, 2015, full-time employees and temporary employees were required to operate a bar feeder, Model - AE EX 110, Serial number  130, on a daily basis.  A chain and sprocket of the Bar feeder was not completely guarded, thereby exposing employees to the hazards associated with moving parts.

29 CFR 1910.132(d)(2): The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.   a.     On or about July 01, 2015, at the above located workplace, full-time employees and temporary employees were required to operate machinery and perform routine tasks that necessitated the use of personal protective equipment, such as eye / hearing protection, gloves, and / or respiratory protection (N95). The employer had not verified, through written certification, that a hazard assessment had been performed, in accordance with this OSHA standard.
