On or about July 1, 2015, on the northeast side of a 6/12 pitch roof of a commercial building, an employee working for Ameritech Enterprises, LLC on the roof was not protected from a 9 feet 5 inch fall hazard by the use of a personal fall protection system.  b.  On or about July 1, 2015, on the north side of a 5/12 pitch roof of a commercial building, an employee working for Ameritech Enterprises, LLC on the roof was not protected from a 9 feet 5 inch fall hazard by the use of a personal fall protection system.

On or about July 1, 2015, on the north side of a commercial building, the ladder used by Ameritech Enterprises, LLC. employees to access the roof did not extend 3 feet above the upper landing surface, exposing employees to an 9 feet 5 inch fall hazard.
