29 CFR 1926.403(i)(2)(i): Except as required or permitted elsewhere in this subpart, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures, or by any of the following means:  In the bathroom for room 202, live parts of electrical equipment operating at 110 to 220 VAC were not guarded against accidental contact, as there was no cover on an electrical panel box 11 inch by 13 inch opening.  The bottom of the panel box was about 64 inches from the concrete floor.  Employees in the bathroom chipping off tile with a jackhammer were exposed to the hazard of contacting electricity.

29 CFR 1926.404(b)(1)(i): The employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites.  These requirements are in addition to any other requirements for equipment grounding conductors:  In room 202, there were no ground-fault circuit-interrupters or assured equipment grounding conductor program to protect employees that used a 110 VAC Black & Decker jackhammer to chip tiles off the bathroom wall.  Power was provided to the jackhammer from a wall outlet and through a flexible cord.  Employees were exposed to the hazard of contacting electricity.

29 CFR 1910.1200(g)(8): The employer did not ensure that material safety data sheets were readily accessible to the employees in their work area during each work shift.  (This provision is applicable to construction employment by 29 CFR 1926.59.):  At the project, there was no safety data sheet for Portland cement.  Employees mixed the cement with water onsite and applied it to the walls as a plaster coating.  Employees were exposed to the hazard of contacting the cement.
