29 CFR 1910.305(b)(2): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy: Garden Shop/Storage: On or about July 1, 2015 and at times prior thereto, the light switch did not have a cover plate. The switch is used daily and exposed employees to potential electrical hazards while using or handling equipment near the doorway.

29 CFR 1910.22(a)(2): The floor of every workroom shall be maintained in a clean and, so far as possible, a dry condition. Where wet processes are used, drainage shall be maintained, and false floors, platforms, mats, or other dry standing places should be provided where practicable: Mechanical Room: On or about July 1, 2015 and at times prior thereto, the workroom floor was wet and slippery from lubrication oil leaking from the landscaping and cutting equipment. The leaking oil onto the floor created a hazard for employees walking into the room. The U.S. Department of Veterans Affairs was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.22(a)(2) which was contained in OSHA inspection number 526479, notice number 2, item number 1 and was affirmed as a final order on November 6, 2012, with respect to a workplace located at 2615 E. Clinton Avenue, Fresno, CA 93703.

29 CFR 1910.305(b)(1)(ii): Unused openings in cabinets, boxes, and fittings was not effectively closed: Machine Shop: On or about July 1, 2015 and at times prior thereto, the main breaker switch has open energized field exposed, showing wiring buss bar. Manufacturer: Siemens Panel Boiler Plant: On or about July 1, 2015 and at times prior thereto, the low water cut out electrical panel switch has open area at the bottom of energized field, exposing employees to potential hazard while using equipment. The equipment is connected to the Honeywell Fluid Gas Valve. The U.S. Department of Veterans Affairs was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.305(b)(1)(ii) which was contained in OSHA inspection number 517118, notice number 2, item number 2 and was affirmed as a final order on September 27, 2012, with respect to a workplace located at 2615 E. Clinton Avenue, Fresno, CA 93703.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) shall be visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket). Cord and plug connected equipment and flexible cord sets (extension cords) which remain connected once they are put in place and are not exposed to damage need not be visually inspected until they are relocated. Engineering Workshop: On or about July 1, 2015 and at times prior thereto, an extension cord and electric light fixture were used without grounding pins to provide general power for shop use. Training/Conference Room: On or about July 1, 2015 and at times prior thereto, extension cords were used with damaged insulation to provide general power for use with television and other electronic equipment used in training.
