Location: 206 Ridge Street, Winchester, MA  On or about 6/19/15, the employer did not ensure that workers utilized the Werner aluminum portable extension ladders at a four to one ratio as required, exposing workers to fall hazards of up to greater than 12-feet, 11-inches.  1. Ladder #1 - 24-foot Werner aluminum extension ladder was at a 65-degree angle. 2. Ladder #2 - 24-foot Werner aluminum extension ladder, model #D1524-2 was at a 64-degree angle. 3. Ladder #3 - 32-foot Werner aluminum extension ladder, model #D1532-2 was at a 60-degree angle.
