29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. The employer did not report the in-patient hospitalization of an employee from a work-related injury within 24 hours to the nearest OSHA area office.

29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. The railing shall be provided with a toeboard wherever, beneath the open sides, At the ClayDesta Building in the mailbox room, an employee worked from an elevated crawlspace in the ceiling of the first floor without fall protection exposing the employee to a fall greater than 4 feet to the ground.

29 CFR 1910.26(c)(3)(vii): Ladders should not be used as a brace, skid, guy or gin pole, gangway, or for other uses than that for which they were intended, unless specifically recommended for use by the manufacturer. At the ClayDesta Building in the mailbox room, employees were exposed to fall hazards, in that the employer did not ensure ladders were used for their intended purpose, such as when a 8 feet A frame ladder was used to access an 11 feet space in the ceiling.
