29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated:      a) Jobsite - Employees operating an extendable boom aerial lift 40 feet above the ground level were exposed to fall hazards without the use of a fall protection system.  Observed on or about 26 MAY 2015.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    a) Jobsite - Employees applying silver coat to the low-slope roof of an outdoor stage were exposed to fall hazards of approximately forty (40) feet without the use of conventional or alternative methods of fall protection. Observed on or about May 26, 2015.
