OSH ACT of 1970 Section (5)(a)(1):   The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a deflagration of propane fuel.  i. The employer was failing to ensure that the tank vehicle containing propane was not parked in a building when the building was not specifically approved for such use by authority having jurisdiction or is designed for such use. NFPA 58 - 1969 Chapter 3.21.c.  ii. Before moving a tanked vehicle into a building for service the system shall be checked for leaks and any found shall be repaired.  NFPA 58 - 1969 Chapter 3.21.d.2.   iii. The vehicle shall not be parked near a source of ignition.  NFPA 58 -1969 Chapter 3.21.d.4.  iv. Unless the product is removed from the cargo tank and pressure reduced to atmospheric, the driver shall instruct the responsible people in the garage of the nature of the cargo contents and instruct them not to tamper with the tank valves or fittings. NFPA 58 - 1969 Chapter 3.21.d.5.  v. Repair work shall not be done on primary shutoff valves while the vehicle is in the garage if the cargo contains product. NFPA 58 - 1969 Chapter 3.21.5.e.  Among other methods, feasible and acceptable methods of abatement are:  Follow procedures and policies set forth by NFPA 58 - 1969 Chapter 3.21.

29 CFR 1910.23(e)(2):   Stair rails did not consist of a top rail, intermediate rail and/or posts, with the vertical height not more than 34 inches (86.36 cm) nor less than 30 inches (76.2 cm) from upper surface of top rail to surface of tread in line with   At the load out station the employer was failing to:   i. Ensure the stair rail on the South side of the steps was protection between the top rail and the platform with an intermediate rail.   ii. On the North side of the were not equipped with a stair rail that consists of a top rail, intermediate rail and/or posts with a vertical height not more than 34 inches nor less than 30 inches.

29 CFR 1910.1200(h)(1):   Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  In the maintenance are of the facility the employer was failing to ensure that maintenance employees were trained on the hazardous chemicals in the work area. Chemicals that the employer failed to train the employee(s) on was but not limited to propane.
