29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grab rail, was not provided to assist employees in mounting and dismounting the ladder:  a) 90 Lee Street, Stewartstown PA; Sunroom - A portable Werner metal extension ladder was used to gain access to the roof and the side rails extended only 7 inches above the landing surface, the ladder was not secured at its top to a rigid support and no grasping device, such as a grab rail was provided to assist employees in mounting and dismounting the ladder, thereby exposing employees to an 8 foot fall hazard to the ground below, on or about June 19, 2015.  No abatement certification or documentation required.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such than the horizontal distance from the top support to the foot of the ladder way approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support):  a) 90 Lee Street, Stewartstown PA; Sunroom - A portable Werner metal extension ladder used to gain access to the roof was set up at an unsafe angle, on or about June 19, 2015. The horizontal distance from the top support to the foot of the ladder exceeded one-fourth of the working length. The foot of the ladder measured 6 feet from the top support and should have measured no more than 2 feet.  No abatement certification or documentation required.
