29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or (c). (a) Turkey Hill Worksite: Employees were working from inside a 5'6 deep, 19'6 by 35'6 excavation to square up and strip concrete forms with an incomplete benching system for cave-in protection for Type B soil where one side of the excavation was not benched, observed on or about June 30, 2015.
