OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to struck-by hazards from a gas operated cut-off saw blade:  WORKSITE: The employer did not conduct a Job Hazard Analysis (JHA) for the task of cutting ductile pipe that was located in the excavation to ensure that the ductile pipe was adequately supported to prevent saw kickbacks during cutting activities.      On or about June 25, 2015, the employee was using a Stihl model TS 420 gas powered cutoff/demo saw to cut a ductile iron pipe in the excavation when the sawblade,  jammed and kicked back and the employee sustained a fatal injury from the incident.
