29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user:   Boiler Plant, Biohazard Autoclave:  On or about June 30, 2015 and at times prior thereto, the employer did not implement medical surveillance or ensure respirators were properly cleaned, stored, and maintained for employees using 3M and AO Safety, half-mask, negative pressure respirators.  This condition placed a physiological burden for voluntary respirator users and exposed users to skin contact hazards when the respirator was donned.

This condition limited employee information of the contents of the container and the use of proper precautions when handling the liquid.
