29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: (a). Worksite located at 200 E. Old Highway 40, Odessa, Missouri- Employees were working in areas in which they could strike their heads on the overhead awning as well as an employee was observed working directly beneath an elevated employee who could drop tools and materials on the employee who was not wearing any means of head protection.

29 CFR 1926.451(g)(4)(iii): When midrails, screens, mesh, intermediate vertical members, solid panels, or equivalent structural members are used, they were not installed between the top edge of the guardrail system and the scaffold platform: (a). Worksite located at 200 E. Old Highway 40, Odessa, Missouri- Employees were exposed to a fall hazard in that the employees were working in a mobile scaffold and did not place the chain over the opening to prevent an employee from falling.
