29 CRF 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury.       A. A & J Construction worksite located at 7622 St. Rt. 109, Delta, Ohio: On or about July 1, 2015, the employer failed to instruct all employees in the recognition and avoidance of hazards commonly seen in the framing industry. Employees were exposed to hazards resulting from the lack of personal protective equipment and fall hazards.

29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.    A. A & J Construction worksite located at 7622 St. Rt. 109, Delta, Ohio: On or about July 1, 2015, the employer did not ensure employees were wearing proper eye protection while operating a pneumatic nail gun when performing framing activities. The employees were exposed to potential flying object hazards while operating the pneumatic fastener.

A. A & J Construction worksite located at 7622 St. Rt. 109, Delta, Ohio: On or about July 1, 2015, the employer did not ensure employees were protected from head injuries while working below employees performing framing activities. Due to this condition, employees were potentially exposed to impact injuries caused from falling material and/or equipment.

A & J Construction worksite located at 7622 St. Rt. 109, Delta, Ohio: On or about July 1, 2015, the employer did not ensure employees were protected from fall hazards while performing framing activities on the West side of the commercial wood frame structure. The exposed employees were not protected from falling approximately 16.00 feet.
