RL Roofing & Repair, LLC worksite located at 8733 N. Stonemill Ct., Sylvania, Ohio: On or about June 29, 2013, the employer did not ensure inspections of the worksite were conducted to ensure employees were not exposed to hazards associated with residential roofing operations (i.e.; fall protection, and use of personal protective equipment).

A. RL Roofing & Repair, LLC worksite located at 8733 N. Stonemill Ct., Sylvania, Ohio: On or about June 29, 2015, the employer did not ensure an employees wore proper eye protection while operating pneumatic nail guns while performing weatherproofing activities. The employees were exposed to potential flying object hazards while operating the pneumatic fasteners.

A. RL Roofing & Repair, LLC worksite located at 8733 N. Stonemill Ct., Sylvania, Ohio: On or about June 29, 2015, the employer did not ensure employees were protected from head injuries while working below employees installing weatherproofing. Due to this condition, employees were potentially exposed to impact injuries caused from falling material and/or equipment.

29 CFR 1926.1053 (b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access.     A. RL Roofing & Repair, LLC worksite located at 8733 N. Stonemill Ct., Sylvania, Ohio: On or about June 29, 2015, the employer did not ensure the side rails of the extension ladders located on the North side of the two story residential structure was extended at least three feet above the landing surface. Employees were exposed to falls while using the ladder to access the roof.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.    A. RL Roofing & Repair, LLC worksite located at 8733 N. Stonemill Ct., Sylvania, Ohio: On or about June 29, 2015, the employer did not ensure employees were protected from falls while installing weatherproofing on the two story residential structure. The employees were exposed to a fall distance of approximately 19.00 feet from the 8/12 pitch roof.  RL Roofing & Repair, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501,which was contained in OSHA inspection number 402142, citation number 1, item number 2 and was affirmed as a final order on April 6, 2013 with respect to a workplace located at Oregon, Ohio.
