29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-sloped roofs with unprotected sides and edges 6 feet (1.8m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line systems and guardrail systems, warning line systems and safety net systems, or warning line systems and personal fall arrest systems, or warning line systems and safety monitoring systems: Along the top roof of the construction building, on or about July 01, 2015 and at times prior there to employee(s) were exposed to a fall of approximately 45 feet to the ground level below while installing roof insulation panels on a 2:12 roof pitch. IN THE ALTERNATIVE: 29 CFR 1926.501(b)(2)(i): Each employee who was constructing a leading edge 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems: Along the top roof of the construction building, on or about July 01, 2015 and at times prior there to employee(s) were exposed to a fall of approximately 45 feet to the ground level below while installing roof insulation panels on a 2:12 roof pitch.
