29 CFR 1910.212(a)(3): Point of operation guard were not provided to protect operator(s) and other employees from hazard created by ongoing nip points:  (a) On or about July 1, 2015 an employee amputated the left index finger while trying to un-jam chicken tenders on a moving cone line.  The splash guard installed on the cone line to prevent product from falling to the floor had a quarter inch opening between it and the moving conveyor. The finger was caught in the quarter inch opening when trying to un-jam chicken tenders with the fingers.
