29 CFR 1926.102(a)(1): Employees shall be provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents.  a. On the roof of the house where an employee was installing roof sheeting to the trusses utilizing an air actuated nail gun, the employee's eyes were not protected, thereby being exposed to struck-by hazards.  b. On the ground where an employee was cutting lumber with a circular saw, the employee's eyes were not protected, thereby being exposed to struck-by hazards.

29 CFR 1926.404(f)(6): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent and continuous.  a. At the worksite where an employee was using an extension cord to provide power to a circular saw, the ground prong was missing from the male end of the extension cord, thereby exposing employees to an 125 volt electrical contact hazard.

29 CFR 1926.405(g)(2)(iv): Strain relief. Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws.  a. At the worksite where an employee was using an extension cord to provide power to a circular saw, the strain relief of the female plug was pulled away from the outer insulation of the cord, thereby exposing employees to an 125 volt electrical contact hazard.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502. Note: There is a presumption that it is feasible and will not create a greater hazard to implement at least one of the above-listed fall protection systems. Accordingly, the employer has the burden of establishing that it is appropriate to implement a fall protection plan which complies with 1926.502(k) for a particular workplace situation, in lieu of implementing any of those systems.  a. On the roof of the house where an employee was installing sheeting, the employee was not protected from falls by a guardrail system, safety net system, or personal fall arrest system, thereby being exposed to an 18 foot fall hazard.  b. On the second story of the house where employees were working near window openings, guardrails were not installed, thereby exposing employees to a 13 foot fall hazard.
