29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:  a. At the Highcroft Ridge Elementary School renovation project, where equipment such as mobile scaffolds were used, regular and frequent inspections were not conducted to identify hazards such as, but not limited to, use of mobile scaffolds near stairway openings, and ensuring that mobile scaffold wheels were locked to prevent injury.  Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).

At the Highcroft Ridge Elementary School renovation project, where equipment such as mobile scaffolds were used, inspections were not conducted of the scaffolds used at the site to identify hazards such as, but not limited to, use of mobile scaffolds near stairway openings, and ensuring that mobile scaffold wheels were locked to prevent injury.  Abatement certification and abatement documentation is required for this violation. 29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner:  a. At the Highcroft Ridge Elementary School renovation project, where equipment such as mobile scaffolds were used, wheels were not locked during use on June 22, 2015; a scaffold rolled into a floor opening during construction work.  Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).
