29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:    A.)  Gerlach's Property Maintenance, LLC, 2727 West 18th Street, Erie, PA  16505, on or about July 2, 2015 and times prior thereto:  Employees have not received safety related electrical work practice training covering items including, but not limited to, GFCI use, grounding, and electrical hazards associated with damaged equipment.    B.)  Gerlach's Property Maintenance, LLC, 2727 West 18th Street, Erie, PA  16505, on or about July 2, 2015 and times prior thereto:  Employees have not received training on the electrical safety related work practice training covered in 29 CFR 1926.416.    C.)  Gerlach's Property Maintenance, LLC, 2727 West 18th Street, Erie, PA  16505, on or about July 2, 2015 and times prior thereto:  Employees have not received training on the lockout tagout requirements covered in 29 CFR 1926.417.    D.)  Gerlach's Property Maintenance, LLC, 2727 West 18th Street, Erie, PA  16505, on or about July 2, 2015 and times prior thereto:  An employee utilizes personal protective equipment including, but not limited to, eye / face protection, steel toed boots and gloves when operating equipment including, but not limited to, a Stihl rotary concrete saw.  The employee has not received personal protective equipment training.    E.)  Gerlach's Property Maintenance, LLC, 2727 West 18th Street, Erie, PA  16505, on or about July 2, 2015 and times prior thereto:  Employees are provided with, and permitted to utilize portable dry chemical fire extinguishers.  Employees have not received fire extinguisher training.

Servicing and maintenance work includes, but is not limited to, checking fluid, adding fluid and lubricating grease fittings on equipment. Gerlach's Property Maintenance, LLC, on or about July 2, 2015 and times prior thereto:  Employees utilize chemicals including, but not limited to, gasoline, grease, Stihl Chain & Saw Lubricant, and Stihl two-cycle engine oil when performing servicing and maintenance work on equipment.  Servicing and maintenance work includes, but is not limited to, checking fluid, adding fluid and lubricating grease fittings on equipment.  Equipment includes, but is not limited to, skid steers, excavators, concrete saws, chains saws, and gasoline blowers.  Employees have not had hazard communication training.
