29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) On or about 7/2/15 at a roofing project located at Lot #33 Crabapple Lane, Lancaster, New York; employees were installing underlayment on a roof with a 6 in 12 pitch and an eave height of approximately 16 feet. There were no fall protection systems in place. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) On or about 7/2/15 at a roofing project located at Lot #33 Crabapple Lane, Lancaster, New York; employees were accessing the roof from the front of the house using a ladder that had the side rails approximately 18 inches above the landing surface. NO ABATEMENT CERTIFICATION REQUIRED
