29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:   a) American Lube Fast, LLC #005 -  Mechanic Area -Employees use degreaser which contains 2- Butoxyethanol, and 45% Caustic Potash to mop and clean floors in the bay area classified as corrosives.  The emergency eye wash provided was broken and was not adequate to flush both eyes simultaneously and the body in case of an emergency, on or about June 3, 2015.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees:  American Lube Fast, LLC #005 -  Employees are exposed to an electrical shock hazard due to the roof leaking in and around electrical equipment in the following areas:  a) Between Bay #1 and Bay #2-Entry side of the light valances; b)Between Bay #1 and Bay #2-Exit of the lights; c)Basement-Near the pit exhaust vent.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  a) American Lube Fast, LLC #005 -  Mechanic Area -Employees were using mechanical and cleaning chemicals such as, but are not limited to, degreaser, washer fluid concentrate, power steering flush, battery protector and terminal spray, and fuel system cleaner.   The employer had not developed or implemented a written hazard communication program for the site, on or about June 3, 2015.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

A Safety Data Sheet (SDS) was not obtained for the Resco Lube & Grease Cleaner used at the site, on or about, June 3,2015.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
