29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle. In the production area, an employee engaged in cutting 1 inch diameter material with a hydraulic operated horizontal band saw was exposed to caught-in and crushed-by hazards when reaching into the pinch point/cutting area.
