29 CFR 1926.501(b)(12): Each employee engaged in the erection of precast concrete members (including, but not limited to the erection of wall panels, columns, beams, and floor and roof "tees") and related operations such as grouting of precast concrete members, who was 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems, and no other provision in paragraph (b) of this section provided for an alternative fall protection measure:  a) Construction site, 5th floor of parking garage - On or about June 19, 2015, an employee walking backwards while pulling welding cables fell off the unprotected edge more than 6 feet to the 4th floor and was seriously injured.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stairrail system along each unprotected side or edge:  a) Construction site, stairway access to upper floors - On or about July 1, 2015, employees used a stairway with more than 4 risers in each flight that did not have a stairrail system installed on the open interior side, exposing employees to a fall hazard.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours:  ) Construction site, pre-cast parking garage - On or about June 19, 2015, an impatient hospitalization occurred after an employee sustained injuries from a fall.  However, the employer did not report the incident until more than 24 hours after the employee's admission.
