29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury. On or about 27 June 2015, the employer failed to provide hazard recognition training for employees removing debris on the first story roof of the building under construction where floor holes were present, exposing employees to hazards such as, but not limited to slips, trips and falls.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. On or about 27 June 2015, the employer failed to provide fall protection for employees removing debris on the first story roof of the building under construction where floor holes were present, exposing employees to a fall hazard.
