29 CFR 1926.502(d)(23): Personal fall arrest systems were attached to guardrail systems or to hoists: Construction site: On or about July 6, 2015 and at times prior thereto, one employee working outside the 3rd floor railing system was attached to the top rail. This condition exposed the employee to a fall greater than 6-feet to ground below.
