29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:  a)  Smith, Dan at 1115 and 1075 Redwood Ave., Boulder, CO 80304:  On or before June 27, 2015, the employer did not ensure that the window well holes were covered to protect subcontractor employees from stepping or falling into them.  Conditions exposed employees to fall hazards of up to 8 feet.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  a)  Smith, Dan at 1115 and 1075 Redwood Ave., Boulder, CO 80304:  On or before June 27, 2015, the employer did not ensure the use of fall protection when framing these single family homes in the following instances:  1)  Subcontractor employees were setting floor joists and sheathing without the us of fall protection at 1115 Redwood Ave.; and  2)  Subcontractor employees were working near the back patio, and the 10' wide wall opening was guarded with only a top rail set at 36", and this guardrail did not conform with the standard guardrail requirements of 42" +/- 3", plus a mid rail, as stated in 29 CFR 1926.502(b).  Conditions exposed employees to fall hazards of up to 9 feet.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches, whichever is less, were not equipped with a stairrail system along each unprotected side or edge:  a)  Smith, Dan at 1075 Redwood Ave., Boulder, CO 80304:  On or before June 27, 2015, the employer did not ensure that a stairway used to access the second floor had a stairrail along the north unprotected edge.  Conditions exposed employees to fall hazards of up to 5'9".
