29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  a)  Smith, Dan at 1115 and 1075 Redwood Ave., Boulder, CO 80304:  On or before June 27, 2015, the employer did not ensure the use of fall protection when framing these single family homes in the following instances:  1)  Subcontractor employees were setting floor joists and sheathing without the us of fall protection at 1115 Redwood Ave.; and  2)  Subcontractor employees were working near the back patio, and the 10' wide wall opening was guarded with only a top rail set at 36", and this guardrail did not conform with the standard guardrail requirements of 42" +/- 3", plus a mid rail, as stated in 29 CFR 1926.502(b).  Conditions exposed employees to fall hazards of up to 9 feet.
