29 CFR 1926.453(b)(2)(iv): Employees shall always stand firmly on the floor of the basket, and shall not sit or climb on the edge of the basket or use planks, ladders, or other devices for a work position. a. On or about July 02, 2015, at the above addressed jobsite, an employee was conducting torching operations while standing on the mid rail of an aerial lift (JLG, 600S), thereby exposing the employee to the hazards associated with swing falls.

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a. On or about July 02, 2015, at the above addressed jobsite, employees working on the third floor performing torching operations were exposed to falls, approximately fifty (50) feet above the ground, created by the presence of unprotected sides.
