29 CFR 1926.405(b)(2): In energized installations, each outlet box did not have a cover, faceplate, or fixture canopy:    a)  Boulder Home Builders LLC at 1115 Redwood Ave., Boulder, CO 80304:  On or before June 27, 2015, the employer provided an an outlet box that was missing its faceplate for subcontractor use to power portable tools.  Condition exposed subcontractor employees to a 120 volt electrical shock hazard.
