29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites:    Employees were exposed to electrical shock hazards when using extension cords which were plugged in to a portable generator. A ground fault circuit interrupter was not used and the employer did not institute an assured equipment grounding conductor program. 29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:    An employee performing carpentry work from a residential roof was exposed to the hazard of free falling more than six (6) feet while using a personal fall arrest system. Rope grabs were not adjusted to protect the employee from a free fall of greater than six (6) feet. The employee was exposed to internal injuries and/or failure of personal fall arrest system. 29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     Employees performing residential construction operations on the roof of a residential property, twelve (12) feet two (2) inches above the ground, were not protected from falling, thereby exposing the employees to fall hazards.    West Builders, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 457453, citation number 1, item number 1 and was affirmed as a final order on July 10, 2012, with respect to a workplace located at 204 Middleton, Elgin, IL 60123.    There is no abatement certification or documentation required for this item.
