29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: Brand Vaughan Lumber Company, Inc., Tucker, GA - Employees were instructed to operate the propane and diesel forklifts in the yard to move lumber. Some of the employees operating forklifts were certified in April 2012 and an evaluation of their performance had not been conducted, on or about 7/6/2015.

29 CFR 1910.178(l)(5): The employer did not evaluate the competency of the operator who had previously received training appropriate to the truck and working conditions encountered. Brand Vaughan Lumber Company, Inc., Tucker, GA - Employees were instructed to operate the propane and diesel forklifts in the yard to move lumber. Some of the employees operating forklifts were previously trained but not evaluated at the site to assure that the operator was competent in the operation of the type of powered industrial truck assigned, on or about 7/6/2015.
