29 CFR 1910.213(l)(2): Boring bits were not provided with guards that would enclose all portions of the bit and chuck above the material being worked. Fairborn USA, Inc. - Upper Sandusky, Ohio: On or about July 7, 2015, employees were exposed to rotating part hazards in that the Central Machinery drill press located in the saw area and associated with the #3580 jump saw was not adequately guarded. The employer did not ensure that the rotating chuck and all portions of the bit were enclosed with a guard.

29 CFR 1910.213(r)(4): Suitable guard(s) were not provided to reduce to a minimum the hazard due to the point of operation of jump saws: Fairborn USA, Inc. - Upper Sandusky, Ohio: On or about June 24, 2015, the employer did not ensure that the jump saw (with equipment ID no. 3580) located in the saw area was guarded to protect employees from amputation injury. An employee operating the saw sustained an amputation injury when the employee's thumb was within the saw's point of operation.
