29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) Prior to setup activities being performed on the horizontal injection molding machinery, the employer did not ensure following steps to de-energization were achieved: (1) Turn off and shut down power to the machinery, (2) Physically locate and operate energy-isolating devices in such a manner as to isolate the machine or equipment from the energy source(s), (3) Affix lockout/tagout devices to each energy isolating device, (4) Verify isolation and de-energization of machinery have been accomplished prior to starting the maintenance and/or servicing work. Setup employees were exposed to struck-by and/or entanglement hazards from unexpected start-up and energizing of horizontal injection molding machinery.
