The midrail chain for the basket entrance was not engaged. The employees were not tied off using a body belt or harness. The employees were exposed to a fall of up to 15'.    Among other methods of abatement, one feasible and acceptable method to correct the hazard is to engage the midrail chain while any employee is in the basket.
