29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a) Greens department: The employer did not develop and implement a program consisting of energy control procedures, training and periodic inspections for equipment serviced or maintained by employees, such as, but not limited to a Vermeer BC1000XL Brush Chipper, on or about 7/7/15.     NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    a) Garage: An eyewash station capable of 15 minutes of continuous flushing was not available where employees used Ecolab CBC Plus Bowl Cleaner to clean the bathrooms, on or about 7/7/15.    b) Green's department office area: An eyewash station capable of 15 minutes of continuous flushing was not available where employees used Ecolab CBC Plus Bowl Cleaner to clean the bathrooms, on or about 7/7/15.    NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent.     Throughout facility: On or about 7/7/15 the employer did not record the following workplace injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2015:  a) Greens Department: On 6/30/15 an employee was struck in the chin by the handle of a tow bed while attempting to remove a mower from the bed.  The employee received stiches and a prescription for pain medication.     NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
