Throughout facility: On or about 7/7/15 the employer did not record the following workplace injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2015: a) Greens Department: On 6/30/15 an employee was struck in the chin by the handle of a tow bed while attempting to remove a mower from the bed. The employee received stiches and a prescription for pain medication. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
