29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:     5 Lancelot Rd, Manalapan, NJ: On or about 07/07/2015 an employee, working alone,  cutting decking / fascia material with a Ridged 10" Dual Bevel Miter Saw received a severe cut / near amputation injury of three fingers of his left hand.  The employer had not provided the employee with instruction in the recognition of this hazard and the methods to avoid and eliminate same.       NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
