On or about 7/6/15, on site, for employees cleaning used mattresses by spraying 35% Hydrogen Peroxide from a 32 ounce spray bottle 2-3 times a week for 20 minutes. The employer required employees to don a 3M 1/2 mask respirators with organic vapor cartridges (Series 5101, 5201, 5301). The employer did not implement a respirator program to include a written program, fit tests, medical evaluation, and training.     a). On or about 7/6/15, on site, for employees dispensing Hydrogen Peroxide, with a manual hand pump, from a 55 gallon drum into individual spray bottles. The employer required employees to don a 3M 1/2 mask respirators with organic vapor cartridges (Series 5101, 5201, 5301). The employer did not implement a respirator program to include a written program, fit tests, medical evaluation, and training.
