Edgemoor St., Wichita, KS 67218 were exposed to fall hazards of 9 feet 3 inches on the 5 in 12 pitch roof, in that employees were not trained effectively to recognize the circumstances when fall protection was required and the means/methods to implement to ensure compliance.
