29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: Well site: On or about July 7, 2015, contractor employees operating a service rig during the work over of an oil/gas well worked from the rig floor. The floor did not have guardrails installed exposing the employees to the hazard of falling over 4 feet.

29 CFR 1910.23(d)(1)(iii): Stairway(s) less than 44 inches wide having both sides open, did not have one stair railing on each side: Well site: On or about July 7, 2015, contractor employees operating a service rig during the work over of an oil/gas well use a set of stairs to access the rig from the ground. The stair did not have railings. This exposed the employees to the hazard of falling from the stairs.
