29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. The railing shall be provided with a toeboard wherever, beneath the open sides:    On July 8, 2015, employees worked inside a commercial sign approximately 30 feet above ground level without fall protection, exposing employees to fall hazards.

29 CFR 1910.67(c)(2)(iv): Employees shall always stand firmly on the floor of the basket, and shall not sit or climb on the edge of the basket or use planks, ladders, or other devices for a work position:    On July 8, 2015, an employee was observed standing on the rails of the bucket of a Wilke Aerial lift, Model:  65XRB:  Serial:  0765XRB144, approximately 30 feet above ground level, exposing employee to fall hazards.

29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift:   On July 8, 2015, at a commercial site, employees were in a basket of a Wilke Aerial lift, Model:  65XRB:  Serial:  0765XRB1446 approximately 30 feet above the ground level without a lanyard attached to the basket, exposing employees to fall hazards.
