29 CFR 1910.305(a)(2)(x): Flexible cords and cables were not protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points. (a) At the loading dock the cords supplying power to the lift device was pinched against the sharp edge of the metal control cabinet where the cable entered the cabinet and the outer insulation jacket was damaged.

29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings were not protected from abrasions: (a) At the loading dock, the conductor cords to the lift device were not protected from abrasions as part of the bushing was missing causing the cords to rub against the sharp edge of the metal control cabinet where the conductor entered the cabinet.
