29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: (Construction Reference: 1926.59) (a) Edge Demolition @ 638 E Colfax Ave. Denver, CO 80204: On or before February 4th 2015, the employer did not ensure that employees received training on the hazards of carbon monoxide and crystalline silica. A propane space heater and a gas-powered concrete saw were used inside a small indoor space with no outside ventilation. This condition exposed employees to elevated levels of carbon monoxide which required emergency medical attention. Employees were also potentially exposed to crystalline silica during concrete cutting operations.

29 CFR 1910.1200(h)(3): "Training." Employee training shall include at least: (i): Methods and observations that may be used to detect the presence or release of a hazardous chemical in the work area; (ii) The physical and health hazards of the chemicals in the work area; (iii) The measures employees can take to protect themselves from these hazards, including specific procedures the employer has implemented to protect employees from exposure to hazardous chemicals, such as appropriate work practices, emergency procedures, and personal protective equipment to be used. (a) Edge Demolition @ 638 E Colfax Ave, Denver CO 80204: On or before February 4th 2015, the employer did not ensure that employees received training on the hazards of carbon monoxide.
