29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed. On or about July 8, 2015, during the installation of gutters on a residential home located in Columbia Station, Ohio 44028, the employer failed to use a A-frame ladder for its intended purpose when: 1. An employee was observed on an A-frame ladder while it was not in the opened position by leaning it against the house. 2. An employee was observed standing on the top step of the ladder that was clearly marked "do not stand at or above this level."

29 CFR 1926.1053(b)(6): Ladders shall be used only on stable and level surfaces unless secured to prevent accidental displacement. On or about July 8, 2015, during the installation of gutters on a residential home located in Columbia Station, Ohio 44028, the employer failed to protect his employees from an approximately 11.5 foot fall when a ladder was not placed on a level surface.
