29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: (a) On or about June 25, 2015, in the Bindery Department of Manufacturing, a Campbell Wrapper Corporation fin seal wrapper (Model: Heritage, S/N: 5645-0371) had guarding which allowed access to the point of operation exposing employees to struck-by hazards. (b) On or about June 25, 2015, in the Bindery Department of Manufacturing, a Campbell Wrapper Corporation fin seal wrapper (Model: Heritage, S/N: 5645-0371) did not have a properly designed and applied control circuitry for the testing or positioning of machine/equipment components, in conjunction with LOTO, to prevent employee exposure to the hazards associated with the movement of machine/equipment components. The jog function in the inch safe mode did not sound an alarm prior to each activation of the equipment.
