29 CFR 1910.1200(h): This provision is applicable to construction employment by 1926.59. 29 CFR 1910.1200(h): Employees were not provided effective information and training as specified in 29 CFR 1910.1200(h)(1) and (2) on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard was introduced into their work area: Worksite located at Main Street and 2nd Street, Madison NE and throughout the company: The employer failed to provide training as prescribed in the paragraph above. Employees are exposed to chemicals such as HTH Chlorine. Abatement certification is required for this violation. The abatement certification sheet is enclosed with the citations.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). Worksite located at Main Street and 2nd Street, Madison NE: The employer has failed to ensure that employees do not enter into excavations which are not protected. At the time of the inspection, employees were observed in an excavation which was not protected as required by the standard. The excavation was approximately 8 feet. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citations.
