29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d)(2):    a)  Limerick Elementary School - On or about July 7, 2015, employees were using corrosive wax strippers such as Devastator and 6PECC3 Baseboard Stripper, and the employer did not perform a required workplace hazard assessment through a written certification which identified the workplace evaluated, the person certifying the evaluation, the date of the hazard assessment, and identification of the document as a certification of hazard assessment.     Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

Abatement Note:  The information in the sharps injury log shall be recorded and maintained in such manner as to protect the confidentiality of the injured employee.  The sharps injury log shall contain, at a minimum:     1)  The type and brand of device involved in the incident.     2)  The dept. or work area where the exposure incident occurred.     3)  Explanation of how the incident occurred.
