29 CFR 1926.404(b)(1)(iii)(C): Each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, shall be visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Equipment found damaged or defective shall not be used until repaired. a. On the 2nd floor hallway where an employees were using an extension cord that was missing a ground prong, the employees did not know to check the cords prior to each day's use, thereby exposing the employee to an electrical contact hazard.

29 CFR 1926.404(f)(6): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent and continuous. a. On the 2nd floor hallway where employees were cutting holes in the framing, the ground prong was missing on the male end of the extension cord, thereby exposing the employee to an electrical contact hazard.
