29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface shall be protected from tripping in or stepping into or through holes (including skylights) by covers. a. In the north side hallway of the first floor where employees were working, holes in the floor were not covered, thereby exposing employees to fall hazards. b. In the east side hallway of the first floor where employees were working, holes in the floor were not covered, thereby exposing employees to fall hazards. c. In the first floor common area where employees were working, holes in the floor were not covered, thereby exposing employees to fall hazards. d. In the first floor outside the general contractors office where employees were working, holes in the floor were not covered, thereby exposing employees to fall hazards.
