Forest Hills, NY Employee undermining pavement in a trench 2 ft. x 3 ft. x 4 ft. in depth was exposed to the hazard of cave-in. Rock fell on the employee's back. ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19.

A competent person was not conducting inspection prior to the start of work. ABATEMENT VERIFICATION IS NOT REQUIRED PURSUANT TO 29 CFR 1903.19.
